Dr. Mitsuo Tomita graduated from UCLA before attending medical school at UC San Francisco.
He then completed his internship and residency in family medicine in Fresno before joining Kaiser Permanente in San Diego in 1975. He retired from full-time family practice at Kaiser in 2008, then did some part-time fill-in work in community health centers in the San Joaquin Valley and in Washington state where he usually spends the summers.
He is an Assistant Clinical Professor on the voluntary faculty staff at the UCSD School of Medicine and is a volunteer for Compassion and Choices, a national non-profit organization that advocates for expanded choices and improved care at the end of life.
A sudden injury or illness can leave one unable to communicate. An Advance Health Care Directive (or Advance Directive) can help you express what what kind of medical care you would want or not want in that situation and who should be relied on for help making those decisions. A POLST or Physician Orders for Life-Sustaining Treatment has replaced the out-of-hospital DNR (Do Not Resuscitate) form. Both will be covered in this presentation.
